According to CURB-65 score
Low severity CAP- oral amoxicillin 5 days
If penicillin allergic- macrolide/tetracycline
Moderate severity CAP- amoxicillin + macrolide 7-10 days
High severity CAP- IV co-amoxiclav/ceftriaxone/piperacillin + tazobactam
CAP-
Atypical- clarithromycin (avoid in long QT syndrome, use erythromycin in pregnant women)
Typical- amoxicillin/co-amoxiclav
Severe- piperacillin-tazobactam
Levofloxacin if cannot tolerate ^
HAP-
<5 days- co-amoxiclav (can cause cholestasis- high BR & ALP)
5 days- tazocin
Pneumocystic jiroveci- co-trimoxazole
Aspiration pneumonia- amoxicillin + metronidazole
MRSA- vancomycin
Legionella pneumonia- oral clarithromycin
